VETERAN STUDENT CHECKLIST

____VA FORM 22-1990 OR 5490: Application for VA Education Benefits or Application for Survivors and Dependent Educational Assistance Form is mailed to VA Regional Processing Office, P.O. Box 8888, Muskogee, OK 74402.

____VA Form 22-1995 or 549:  Request for Change of VA Education Program or Place of Training form is required for transfer students or change of major.

____DD Form 214: For veterans, Member 4-copy is required to establish initial eligibility for educational benefits.  Delimiting date for Montgomery GI Bill benefits is ten years from date of separation, with 36 months of full time educational benefits.

____Degree Plan: Academic Advisor will provide a degree plan once student has made appointment and all transcripts have been received and evaluated, including former schools and military transcript. Students must provide a new degree plan for change of major.

____Veterans Intent to Enroll: Texas A&M University form must be completed at the beginning of each semester, as soon as veteran has registered for classes. Internal form updates student information, benefit type, major, and number of hours student has enrolled in. Veteran students must complete this form each semester to be certified for benefits.

____Military Transcripts: For transfer credit evaluation of military coursework and experience, select Texas A&M University-Corpus Christi when ordering military transcripts on the following websites: How To Obtain Military Transcripts

It is the responsibility of all students receiving veterans educational benefits to inform the VA office of any changes, including added or dropped courses, withdrawals, or change of major.
